Patients or SDMs are responsible for managing their care providers, managing funding, and reporting to Ontario Health atHome. This program offers flexibility and independence while ensuring equitable care.

Who is Eligible for Family-Managed Home Care?

Eligibility for the FMHC program is determined by Ontario Health atHome and includes the following requirements:

• Patients must meet the criteria for traditional home care and have a care plan developed by Ontario Health atHome.
• SDMs must hold Power of Attorney, be a Guardian of Property, or manage other programs funded by the Ministry of Children, Community and Social Services (MCCSS), such as Passport, Ontario Works, or the Ontario Disability Support Program.
• For patients under 18, the SDM can be a parent or individual with legal custody.
• Patients must fall into one of these eligible groups:
  • Children with complex medical needs
  • Adults with acquired brain injuries
  • Patients educated at home
  • Patients in extraordinary circumstances
